An employee suffered a serious injury resulting in multiple finger amputation on his right hand.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) On or about March 15, 2024, and at times prior, employees were not provided with specific information regarding the types and magnitude of hazardous energy, potential hazards and the means to perform a full and effective removal of hazardous energy present in the Hagan Box Machine when performing tasks such as but not limited to clearing jams. This exposed employees to amputation and caught-in between hazards.

Employees are routinely unjamming the Hagan Box Machine on a daily basis without applying lockout/tagout devices.
