29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) Midway Wholesalers, Inc. On or about March 20, 2024, the employer did not ensure that the exit route door at the east exit of the warehouse locks only from the outside. Failure to have such a locking device installed on exit door(s) can limit employees ability to escape the facility in the event of an emergency, potentially exposing them to fire hazards.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck. a) Midway Wholesalers, Inc. on or about March 20, 2024, the employer failed to ensure that employees operating powered industrial trucks were adequately trained, evaluated, and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.
