29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about March 20, 2024, at the snacks vending area. Electrical equipment such as relocatable power tap (RPT) and extension cord were used in place of fixed firing to provide power to various kitchen appliances, exposing employees to electrical and fire hazards as follows:   a. A seven-outlet relocatable power tap/power strip surge protector, Commercial Electric, model YLPT 45-2 (15- amp) was used to provide power to kitchen appliances such as toaster oven (12.5-amp); Omcan coffee percolator (12.5-amp) and Nutty Bavarian NBR2000 Plus electric peanut roaster (15.4-amp).  b. A 20-ft extension cord was used to provide power to a Coca-Cola beverages cooler.   NOTE: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  On or about March 20, 2024, electrical receptacles used as power source were not fully-covered with a face plate cover as follows:  a. At the lunch break area, the two-outlets electrical receptacle available for use had its cover lose, exposing conductors.   b. At the snacks area, the two-outlets electrical receptacle used for to connect an extension cord for the kitchen appliances had its cover lose, exposing conductors.    NOTE 1: Certification of abatement is not required for Instance (a) since it was submitted prior to citation issuance.   NOTE 2 : Certification and documentation of abatement must be submitted for Instance (b) this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries  On or about March 20, 2024, fully-charged Class ABC 5-lbs portable fire extinguishers were not mounted as follows:  a. At the snacks bar area next to the Coca-Cola cooler, portable fire extinguisher placed at floor level behind trash can.   b. At the main cash registers area, portable fire extinguisher placed at floor level behind merchandise rack.  c. At the main cash registers area, portable fire extinguisher placed on top of counter behind merchandise display rack.   d. At the main entrance next to an exit door, a wall-mounted portable fire extinguisher was obstructed by marine metal decorations such as two ship bells.   NOTE: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.
