29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: a)Talco Construction, LLC at 645 North Main Street, Spearfish, SD 57783: On or about and at times prior to March 20, 2024, the employer did not ensure an accident prevention program was developed and implemented for the work environment to include regular inspections of the jobsite, materials, and equipment; conducted by a competent person. Employees were exposed to fall hazards from the roof and awning areas, as well as a lack of training regarding scaffolding and ladders being used on site while conducting commercial roofing operations. This condition exposed employees to a variety of hazards identified on site.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a)Talco Construction, LLC at 645 North Main Street, Spearfish, SD 57783: On or about and at times prior to March 20, 2024, the employer did not ensure that employees were protected from falls while conducting roofing work on a commercial property by an adequate fall protection system. This condition exposed employees to fall hazards greater than six feet.
