Subcontractor employees were conducting re-roofing operations on a federal building with a roof pitch of 5-in-12 and up to  8-in-12 while not utilizing a fall protection system. This condition exposed subcontractor employees to a fall hazards from 10'7" to 28 feet.

This condition exposed employees and subcontractor employees to a fall hazards.  (b)RJS Construction, Inc. at 2014 North Main Avenue, Spearfish, SD 57783: On or about and at times prior to March 20, 2024, the controlling employer did not ensure that the ladder on the west side of the building north of the front entrance being used for access onto the roof of a federal building was extended at least three feet above the upper landing surface of the roof.  This condition exposed employees and subcontractor employees to a fall hazards.
