Employees were conducting re-roofing operations on a federal office building with a steep pitched roof of 5-in-12 and up 8-in-12 while not utilizing a fall protection system. This condition exposed employees to a fall hazards greater than six feet.

This condition exposed employees to a fall hazards.  (b)Sergio Flores at 2014 North Main Avenue, Spearfish, SD 57783: On or about and at times prior to March 20, 2024, the employer did not ensure that the ladder on the west side of the building north of the front entrance being used for access onto the roof of a federal building was extended at least three feet above the upper landing surface of the roof.  This condition exposed employees to a fall hazards.
