Location: a) On the staircase from the first floor to the basement level: On or about March 20, 2024, an employee was welding a metal staircase handrail where he did not use any welder's masks for protection against injurious light radiation. The employee was exposed to radiation energy hazard.

Location: a) On the staircase from the first floor to the basement level: On or about March 20, 2024, employees were using a portable hand grinder to weld install a metal staircase handrail. The portable hand grinder did not have a guard when used.
