1910.179(n)(2)(ii):  The load shall be attached to the load block hook by means of slings or other approved devices.   At an establishment located at 5107 County Road C, in Manitowoc, Wisconsin;   (a) On or about March 20, 2024, in Bay 4, the JTEKT, Type:  FH800Sx-1, Machine No:  NS-5965 workstation utilized an inhouse Ladder Frame Lifting Device, S/N: T6401 to move parts weighing 350 to 500 pounds in and out of the machine was rated nor marked exposing employees to struck by hazards.  (b)  In March 2024, in Bay 4, the Toyoda Mitsui Seiki, H630AP, FH630SX-High Torque Spindle CNC workstation utilized an inhouse lifting device to move parts weighing approximately 52 pounds in and out of the machine was not rated nor marked exposing employees to struck by hazards.    (c)  In March 2024, in Bay 4, the Mori Seiki NH5000 DCG, Model:  NH5000DCG150, S/N:  NH502120913, workstation utilized an inhouse lifting device to move parts in and out of the machine was not rated nor marked exposing employees to struck by hazards.

29 CFR 1910.178(o)(1):  Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered.  On or about March 5, 2024, at an establishment located at 5107 County Road C, in Manitowoc, Wisconsin; in Bay 2 a forklift operator was traveling down the main aisle with a metal cart on the forks of the Nissan electric forklift, Model:  CK1B1L155, S/N:  CKB1001287, the cart shifted exposing employees to struck by hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  At an establishment located at 5107 County Road C, in Manitowoc, Wisconsin;   (a)  On or about March 20, 2024, in Bay 4, the Mitsubishi MHT-11 HBBAC horizontal boring machine used to make fixtures exposed employees to struck by hazard from the inadequate and/or unguarded    1)  at the tool carriage,    2)  pig tail auger, and    3)  moving table.  (b)  On or about March 26, 2024, in Bay 1, New London Engineering conveyor, Model:  800-30"-12'0", S/N:  124220, had an unguarded in running nip point on the outfeed metal chain conveyor exposing employees to caught between hazards when grabbing parts of the line.   (c)  On or about March 20, 2024, in Bay 4, the key controlling the door interlock for the Mori Seiki MH-1000, S/N:  27 was left in the machine and allowed an employee to turn off the interlock function exposing employees to struck by hazards.
