29 CFR 1910.23(e)(1)(vii): The employer did not ensure that mobile ladder stands and platforms that have wheels or casters are equipped with a system to impede horizontal movement when an employee is on the stand or platform. a) 201 W Kendig Road, Willow Street, PA - On or about March 21, 2024, employees conducted work from a three-step mobile ladder stand that was missing the rubber tips from the legs allowing the horizontal movement of the ladder when employees are on the stand platform. b) 201 W Kendig Road, Willow Street, PA - On or about March 21, 2024, employees conducted work from a nine-step mobile ladder stand that was missing the rubber foot pads from the legs allowing the horizontal movement of the ladder when employees are on the stand platform. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: a) Dry Ice Production Area - On or about March 21, 2024, employees were exposed to a caught-in hazard from an unguarded rotating sprocket wheel and chain on the horizontal shaft driving the belt conveyor within an opening at a height of 4 inches to 38 inches above the floor. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
