29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation: On or about March 21, 2024, the employer failed to ensure employees were adequately protected from cave in hazards associated with water accumulation in a trench while replacing a section of pipe. Water was accumulated in the trench due to a leakage in a section of water line.
