29 CFR 1910.147(c)(7)(i)(A): The employer failed to provide adequate training to their authorized employee(s) in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a. Extrusion Line 8801: Conair Puller 6-39 S/N: 236244491434 - On or about March 19, 2024, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. Machine set-up operators were exposed to caught-in hazards to their hands and fingers while removing film that was entangled around the puller's conveyor belt.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Conair Puller 6-39 S/N: 236244491434 - On or about March 19, 2024, employees were exposed to machine hazards associated with moving parts, pinch-points, and crushing hazards during servicing to unjam plastic film at the Extrusion Line 8801 area. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)](i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
