29 CFR 1910.28(b)(13)(i): The employer did not ensure that when work is performed less than 6 feet (1.6 m) from the roof edge, each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system. On or about March 21, 2024, and at times prior there to, in the roofing department, employees were performing roofing work activities without fall protection while working on the roof of a manufactured home greater than four (4) feet to the next lower level.

29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that midrails are installed at a height midway between the top edge of the guardrail system and the walking-working surface. On or about March 21, 2024, and at times prior there to, in the roofing department on the west side of a mezzanine near an air hose, employees used the mezzanine walking/working surface to access a scaffold and the mezzanine guardrail system did not have a midrail.
