29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about March 21, 2024, employees of Imagine Better LLC were performing roofing work at 1149 Crystal Creek Parkway in Wentzville, Missouri. The employer did not ensure eye protection was utilized by employees operating pneumatic nail guns. Employees were exposed to struck-by hazards from ricochets, broken wood, and other debris.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On or about March 21, 2024, employees of Imagine Better LLC were performing roofing work at 1149 Crystal Creek Parkway in Wentzville, Missouri. The employer did not ensure that fall protection was worn while work was being performed on the roof approximately 11 feet above the lower level. The employees were exposed to fall hazards to the ground below.
