On or about March 21, 2024, employees of Neftali Miqueas Tecu Raymundo, dba: Neftali Raymundo were performing roofing work at 267-273 Savannah Terrace in Wentzville, Missouri. The employer did not ensure protective helmets were utilized by employees performing ground clean-up. An employee gathering up old roofing materials, and other debris, near the garage of 271 Savannah Terrace did not don a protective helmet when exposed to falling objects from above.

The employer did not ensure eye protection was utilized by employees operating pneumatic nail guns. Employees were exposed to struck-by hazards ricochets, broken wood, and other debris.
