Lisbon, WI,  the Werner extension  ladder erected on the East side of the structure, at the front door, being used for access to the 8:12 pitched roof, did not extend at least three (3) feet above its upper contact point, and was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately 17 feet, 4 inches onto a gravel covered surface below.
