29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (a) On or about March 21, 2024, at a new construction residential jobsite located at N59W27401 Autumn Ct. Lisbon, WI, the employer did not ensure the competent person was conducting frequent and regular inspections of the job site to ensure that hazardous conditions were identified and corrected. Hazards identified included but were not limited to: fall protection not being worn, fall protection systems were not properly installed,  ladders were not installed and/or used properly, personal protective equipment was not being worn to include safety glasses when required.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about March 21, 2024,  at a new construction residential jobsite located at N59W27401 Autumn Ct. Lisbon, WI, employees operating pneumatic nail guns while roofing a residential structure were not wearing safety glasses. This  exposed employee(s) to a struck by / impalement hazard to the eyes.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph         1926.501 (b):  (a) On or about March 21, 2024, at a new construction residential jobsite located at N59W27401 Autumn Ct. Lisbon, WI,  employees conducting roofing work on a 8:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 25 feet onto dirt, gravel, and building material on the South side of the structure and 17 feet 4 inches on the East and West sides of structure.

(a) On or about March 21, 2024, at a new construction residential jobsite located at N59W27401 Autumn Ct. Lisbon, WI, the employer did not provide fall protection training to recognize fall hazards and implement fall protection methods while performing construction work above six  (6) feet in height.

29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) On or about March 21, 2024, at a new construction residential jobsite located at N59W27401 Autumn Ct. Lisbon, WI,  the Werner extension  ladder erected on the East side of the structure, at the front door, being used for access to the 8:12 pitched roof, did not extend at least three (3) feet above its upper contact point, and was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately 17 feet, 4 inches onto a gravel covered surface below.
