Avondale, PA: On or about March 21, 2024, employees performed tasks associated with removing mushrooms from the second level of the Mushroom House. Four employees performed these tasks while standing over an open hole in the floor of the second level measuring approximately 7 feet to the ground. Employees were not provided with a means of fall protection exposing employees to fall hazards from unprotected edges.
