29 CFR 1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii). Jobsite located at or near 3014 West Lehigh Ave, Philadelphia, PA.- The employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. Employees conducting masonry activities were utilizing scaffold platforms that was not fully planked, thereby exposing employees to fall hazards of a) Approximately 13 feet. b) Approximately 15 feet. Condition was observed on or about March 22, 2024.

29 CFR 1926.451(e)(1):When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access. a) Jobsite located at 3014 West Lehigh Ave, Philadelphia, PA.- The employer did not ensure that employees were provided a safe means to access and egress the supported tubular welded frame scaffold. Employees climbed the scaffold end frames and cross braces to access working levels of the scaffold. Employees were exposed to falls of approximately 13 and 15 feet. Condition was observed on or about March 22, 2024.

29 CFR 1926.451(g)(1)(vii):For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. Jobsite located at 3014 West Lehigh Ave, Philadelphia, PA.- The employer did not ensure employees conducting masonry activities from tubular welded frame scaffold were provided fall protection systems or guard rail systems: a) Employees working on the 2nd level of the scaffold measuring at height approximately 13 feet from the ground level. b) Employees working on the 3rd level of the scaffold measuring at height approximately 15 feet from the ground level. Condition was observed on or about March 22, 2024.
