The employer did not inspect catwalk on a regular basis exposing employees to falls and struck by injuries.

29 CFR 1910.140(c)(20): Ropes, belts, lanyards, lifelines, and harnesses used for personal fall protection must be protected from being cut, abraded, melted, or otherwise damaged. a) Westbound Interstate 70 at Wheeling Tunnel; On or about March 22, 2024 and times prior, employees used personal fall arrest systems while installing an advertisement on a billboard sign. The nylon twin leg lanyards and cloth harnesses used were exposed to cuts, abrasions and other damage from an unprotected sharp edge of an expanded metal catwalk.
