29 CFR 1910.215(b)(9):The employer did not ensure an adjustable tongue guard for the abrasive wheel machinery where an operator stands in front of the machine was installed to allow for adjusting the guard to the constantly decreasing diameter of the wheel. The distance between the wheel periphery and the adjustable tongue guard at the top shall never exceed 1/4-inch. On or about March 21, 2024, and times prior thereto, north side of the machine shop, the employer did not ensure adjustable tongue guards were installed prior to employees operating the bench grinder, exposing them to struck-by flying objects.
