29 CFR 1910.22(d)(2): The employer did not ensure hazardous conditions on walking-working surfaces were corrected or repaired before an employee used the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. In buildings A and B, at Ryder Systems, in Topeka, Kansas, employees engaged in forklift and shipping operations were exposed to struck-by and trip and fall hazards. Employees were exposed in that walking-working surfaces where forklifts travel had damage that could alter forklift travel paths and cause unsecured loads to become a hazard to employees. In addition, walking-working surfaces where employees walk contained broken and cracked concrete, in excess of 2 inches in multiple locations throughout facility causing a tripping hazard.
