29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) For the Homag Centateq N-500 CNC Machine, on or about 03/25/2024, the employer exposed employees to potential laceration and amputation hazards, in that, procedures were not developed, documented and utilized  for the control of potentially hazards energy such as rotating machining bits and moving machine parts.  b) For the Altendorf WA8 sliding table saw, on or about 03/25/2024, the employer exposed employees to potential laceration and amputation hazards, in that, procedures were not developed, documented and utilized  for the control of potentially hazards energy such as rotating saw blades.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) For the Homag Centateq N-500 CNC Machine, on or about 03/25/2024, the employer exposed employees to potential laceration and amputation hazards, in that, authorized employees were not trained to apply locks and tags to energy isolating devices prior to conducting maintenance/servicing activity such as, but not limited to, greasing of the machine, changing bits and cleaning the machine.  b) For the Altendorf WA8 sliding table saw, on or about 03/25/2024, the employer exposed employees to potential laceration and amputation hazards, in that, authorized employees were not trained to apply locks and tags to energy isolating devices prior to conducting maintenance/servicing activity such as, but not limited to, changing saw blades.

29 CFR  1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:   a) For the Homag Centateq N-500 CNC Machine, on or about 03/25/2024, the employer exposed employees to potential laceration and amputation hazards, in that, authorized employees routinely greased the machine, changed bits and performed cleaning activities as part of maintenance and servicing without placing locks and tags on the two energy isolating disconnects.   b) For the Altendorf WA8 sliding table saw, on or about 03/25/2024, the employer exposed employees to potential laceration and amputation hazards, in that, authorized employees routinely changed out saw blades as part of maintenance and servicing without placing locks and tags on the saw's energy isolating disconnect.
