(a) On or about March 25, 2024, the employer did not ensure that a Bliss press (Model # C45 asset number 45BA) was adequately guarded. The metal guard did not prevent employees from reaching through the guard to reach the point of operation via the approximately 1 1/2-inch gaps on the sides. This exposed employee(s) to a crushing and amputation hazard. (b) On or about March 25, 2024, the employer did not ensure that a Bliss press (Model # C45 asset number 45BC) was adequately guarded. The metal guard did not prevent employees from reaching through the guard to reach the point of operation via the approximately 1 1/2-inch gaps on the sides. This exposed employee(s) to a crushing and amputation hazard. (c) On or about March 25, 2024, the employer did not ensure that a Bliss press (Model # 21 1/2M asset number 60BG) was adequately guarded. The metal guard did not prevent employees from reaching through the guard to reach the point of operation via the approximately 2-inch gaps on the sides. This exposed employee(s) to a crushing and amputation hazard. (d) On or about March 25, 2024, the employer did not ensure that a Bliss press (Model # 21 1/2M asset number 60B) was adequately guarded. The metal guard did not prevent employees from reaching through the guard to reach the point of operation via the approximately 2-inch gaps on the sides. This exposed employee(s) to a crushing and amputation hazard. (e) On or about March 25, 2024, the employer did not ensure that a Bliss press (Model # 21 1/2M asset number 60BA) was adequately guarded. The metal guard did not prevent employees from reaching through the guard to reach the point of operation via the approximately 2-inch gaps on the sides. This exposed employee(s) to a crushing and amputation hazard. (f) On or about March 25, 2024, the employer did not ensure that a Bliss press (Model 75-ton asset number 75BB) was adequately guarded. The metal guard did not prevent employees from reaching through the guard to reach the point of operation via the approximately 2-inch gaps on the sides. This exposed employee(s) to a crushing and amputation hazard. (g) On or about March 25, 2024, the employer did not ensure that a Bliss press (Model # 28 asset number 90BA) was adequately guarded. The metal guard did not prevent employees from reaching through the guard to reach the point of operation via the approximately 2-1/4-inch gaps on the sides and an approximately 2 1/2-inch gap in the front slot. This exposed employee(s) to a crushing and amputation hazard. (h) On or about March 25, 2024, the employer did not ensure that a Bliss press (Model # 28 1/2 asset number 125AB) was adequately guarded. The metal guard did not prevent employees from reaching through the guard to reach the point of operation via the approximately 5x5 inch hole in the front of the guard and the approximately 1 3/4-inch gaps in the sides. This exposed employee(s) to a crushing and amputation hazard.
