29 CFR 1910.145(c)(3): Safety instruction signs were not used where there was a need for general instructions and suggestions relative to safety measures: (a) B Pod Unloader Conveyor - B Pod unloader conveyor 345 did not have any safety signs instructing employees to keep clear of the conveyor's moving parts, including the belt and shafts on the conveyor's underside, and instructing employees to use lockout/tagout before performing servicing and maintenance, exposing employees to caught-in and entanglement hazards, on or about March 7, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

1910.147(f)(1)(v) All systems were not de-energized and energy control measures reapplied in accordance with paragraph (d) of this section to continue the servicing and/or maintenance: (a) 4825 Gettysburg Road, Mechanicsburg, Pennsylvania - While troubleshooting mechanical problems on unguarded machinery and equipment, including, but not limited to, the driven horizontal shaft on B Pod unloader conveyor 345, employees did not turn off the unguarded machinery and equipment [per 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], lockout/tagout device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], did not get implemented to protect employees from equipment servicing hazards, such as getting caught-in moving machine parts, on or about March 7, 2024. (b) 4825 Gettysburg Road, Mechanicsburg, Pennsylvania - Employees performed some servicing and maintenance work on machinery and equipment, including, but not limited to, replacing the flight chain on the Douglas case packer, by turning off the machinery and equipment, turning a single Allen-Bradley Prosafe key to off, and removing the single key (trapped key system, which comprises control circuitry) rather than by isolating all energy sources, including, but not limited to, electrical and pneumatic energy, via energy isolating devices [(d)(3)]; applying lockout/tagout to all energy isolating devices [(d)(4)]; dissipating residual energy [(d)(5)(i)]; and verifying isolation [(d)(6)]; exposing employees to equipment servicing hazards, such as getting caught-in moving machine parts, on or about March 22, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
