29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about March 25, 2024 at a work location located at 3777 Boettler Oaks DR, Uniontown Ohio 44685 the employer did not ensure that employees installing shingles on a commercial location were protected from flying debits while installing roofing material with air driven nail guns, hand tools, and leaf blowers. The employees were actively engaged installing material and did not utilize eye and or face protection.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about March 23, 2024 at a work location located at 3777 Boettler Oaks DR, Uniontown Ohio 44685, the employer did not ensure that employees engaged in roofing activities were protected from falling from a height of approximately 21.1 feet with a pitch of 8:12. The employees were in the process of installing roofing material on a commercial roof without the use of fall protection and or fall protection system.
