29 CFR  1926.1053(b)(1): Because of the portable ladders' length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that will not deflect, and grasping device, such as grab rails, shall be provided to assist employees in mounting and dismounting the ladder.  a) On or about March 26, 2024, jobsite located at 1428 Alton Rd. Miami Beach FL. 33139, the employer did not ensure the 24-foot aluminum extension ladder being used to access the roof from the lower level were secured at the top to a rigid support so that the ladders would not deflect while employees using the ladder that was not secured to access a roof were exposed to fall hazard of approximately 20-foot fall.

Miami Beach FL. 33139, the employer did not maintain a copy of the written hazard communication program at the jobsite for employees who are exposed to hazardous chemicals including, but not limited to, Rust Oleum Spray paint and Karnak amphibikote.
