29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards to prevent employees from general slip or trip hazard:  Employees performing daily operations in the back room and the main showroom area were exposed to fall, slip or trip hazards in that the following appliances at the following locations were plugged into the yellow and orange extension cords running on the floor. a. Back room - A flexible cord running from the South wall outlet to the hotbox was utilized to energize the two heaters inside the hotbox. b. Main showroom - A flexible cord was plugged in the outlet running from the main store outlet laying on the floor through the door leading to the back room to power the two heaters inside the hotbox.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for fixed wiring of a structure:  Employees performing daily operations in the back room and the main showroom area were exposed to electrical shock, and fire hazards in that the following appliances at the following locations were plugged into the yellow and orange extension cords in lieu of permanent wiring. They were using as a substitute for the fixed wiring of a structure. a. Back room - A flexible cord running from the South wall outlet to the hotbox was utilized to energize the two heaters inside the hotbox. b. Main showroom - A flexible cord was plugged in the outlet running from the main store outlet laying on the floor going through the door leading to the back room to power the two heaters inside the hotbox.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors:  Employees performing daily operations in the back room and the main showroom area were exposed to electrical shock, and fire hazards in that the following appliances at the following locations were plugged into the yellow and orange extension cords. Employees were permitted to use flexible cords, used to provide power to two heaters plugged into outlets, that was routed through the holes of a wooden wall of the hotbox thus providing no protection from abrasive to the cords. a. Back room - A flexible cord going from the South wall to the hotbox was run through the hole of the wooden walls to energize the two heaters inside the hotbox. b. Main showroom - A flexible cord was run through the hole of the wooden walls, which plugged in the outlet running from the main store floor through the door leading to the back room to power the two heaters inside the hotbox.

Materials or equipment were placed, either permanently or temporarily, within the exit route:   Employees engaged in daily operations in the back room were exposed to entrapment hazards in that the exit was blocked by the temporary storage of mattresses, bed frames, file cabinet, draws, nightstand, blocking the route leading to the exit door, exposed to entrapment hazards while working in the back room, preventing employees evacuating through that exit door in case of emergency, as the employer failed to ensure the exit door remained unobstructed.
