Roof - On or about March 26, 2024, employees performing patch roof repairs to a two story above grade apartment building were not protected from fall hazards by a fall protection system, employees were exposed to a fall of approximately 18 feet. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. Roof - On or about March 26, 2024, employees were using a 28-foot Werner fiberglass extension ladder to access the roof, the ladder was not extended three feet above the landing, employees were exposed to a fall of approximately 18 feet. No abatement certification or documentation required.
