29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: On or about March 20, 2024, the employer failed to ensure the Allen Six Spindle Drill Press was turned off or shut down prior to changing out drill bits. The employer allowed the use of a "magic chuck" in order to change drill bits with the drill spindle turning.
