29 CFR 1928.57(a)(6)(iii): The employer did not ensure to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment: An employee engaged in corn flaking operations located at 73027 K Road, Holdrege, Nebraska 68949 was exposed to amputation hazards in that the corn flaking machine was not fully stopped nor was the power source isolated prior to attempting to remove a foreign object from the corn flaker. As a result, an employees hand was amputated. This was most recently documented on or about March 27th, 2024.

29 CFR 1928.57 (a)(6)(v): At the time of initial assignment and annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he is or will be involved, including locking out electrical power before performing maintenance on farmstead equipment: An employee engaged in corn flaking operations located 73027 K Road, Holdrege, Nebraska 68949 was exposed to the unexpected release of energy sources such as electrical, hydraulic, and steam in that the employee did not receive lockout training at the time of his initial assignment or prior to preforming work on the corn flaker machine. This was most recently documented on or about March 27th, 2024.

29 CFR 1928.57(c)(4)(ii)(B)(2): The employer did not ensure the employees not remove the guard or access door until all components have stopped. An employee engaged in corn flaking operations located at 73027 K Road, Holdrege, Nebraska 68949 was exposed to an amputation hazard in that the corn flaker internal rotating part did not come to a complete stop prior to an employee accessing and removing a guard on the corn flaker. This was most recently documented on or about March 27th, 2024.
