This was most recently documented at 6303 Q Street, Omaha, Nebraska on or about March 24, 2024. The employer failed to ensure that a forklift operator was retrained after an accident. The powered industrial truck being operated by the employee was a JLG 864 All-Terrain Forklift, SN: 160089213.
