29 CFR 1910.178(m)(4):The employer shall prohibit arms or legs from being placed between the uprights of the mast or outside the running lines of the truck. A) 1000 Floral Ave, North Union, NJ 07083 The employer failed to ensure that operators keep both legs on the platform while in operation. The operator of the Hyster electric walkie rider stuck his leg out while turning a corner in the warehouse. His leg was struck by the protector bumper that protects the wall. This exposed the employee to a struck by hazard. On or about 3/27/24

29 CFR 1910.178(l)(1)(i):The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). A) 1000 Floral Ave North, Union NJ 07083 The employer did not have a documented training records from the trainer that the employee(s) operating a forklift (such as the Hyster Walkie Rider) were competent and had been evaluated when the training was completed. Documented records to include, but not limited to: (1)The name of the operator, (2) the date of the training, (3) the date of the evaluation, (4) the identity of the person(s) performing the training or evaluation and (5) signed certificate of certification. Condition(s) were noted on or about March 27, 2024
