29 CFR 1926.20(b)(2): The employer did not ensure that frequent and regular inspections of the job sites, materials, and equipment were made by competent persons designated by the employers:   a) Job site, 7020 Ohio River Boulevard, Ben Avon, PA: On or about March 28, 2024, during roofing activities, the employer did not ensure that a system was in place to identify and correct safety and health issues. Deficiencies noted at the site included, but were not limited to, not using fall protection where exposed to falls of approximately 8 feet or so, not using safety glasses when operating a nail gun, failing to ensure that a worker who worked below an employee on the roof used appropriate head protection, not ensuring that the walk/work area was free of clutter, failing to ensure that ladders had legible safety labels and allowing an extension cord with a cut in the outer jacket to be used.
