Production - On or about April 4, 2024, the employer did not adequately assess what personal protective equipment was required in the facility, including eye and hand protection, to protect affected employees against contact with chemicals such as liquid smoke concentrate.  b. Sanitation - On or about March 28, 2024, the employer did not adequately assess what personal protective equipment was required in the facility, including eye, face, and hand protection, to protect affected employees against contact with chemical splashes or skin burns from use of chemicals such as 45642 General Purpose Cleaner (an irritant), ZEP FS Formula 386L No-Rinse Acid Cleaner Sanitizer (a corrosive), 5118 Smokehouse Cleaner (a corrosive), 3022 Foaming Chlorinated Cleaner (a corrosive), and 9929: HD Foaming Acid Cleaner (a corrosive).

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. Production - On or about April 4, 2024, employee(s) were exposed to chemical splash hazards to their eyes and face from liquid smoke concentrate spraying while loading sausages into the racks on the liquified smoke machine.  b. Sanitation - On or about March 28, 2024, employees were exposed to chemical splash hazards and skin burns to their eyes and face when not wearing adequate eye and face protection while using 45642 General Purpose Cleaner (an irritant), and ZEP FS Formula 386L No-Rinse Acid Cleaner Sanitizer (a corrosive), to perform sanitation activities such as filling diluted mixtures into portable stainless tubs and using a hose to spray the foam and sanitizer chemicals.

Sanitation - On or about March 28, 2024, employee(s) were exposed to chemical burns and splash hazards to their hands from dermal contact with the 45642 General Purpose Cleaner (an irritant) and ZEP FS Formula 386L No-Rinse Acid Cleaner Sanitizer (a corrosive) when performing sanitation activities such as foaming and sanitizing at the ZEP 517/LC Hose Drop Station.

Facility Wide - On or about March 28, 2024, the employer did not ensure an energy control program consisting of an energy control procedure, employee training and periodic inspections was established for employees performing service and maintenance on machines including, but not limited to, the chopper, mixer, liquid smoker, and clipper machines.  The employees were thereby exposed to hazards associated with pinch points and rotating parts.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:   a. 1st Floor Production/Basement Production & Packager - On or about March 28, 2024, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage, and removal of energy control devices. Machine operators were exposed to caught-in hazards affecting their hands and fingers while performing sanitation/cleaning activities on the semi-automatic clipper and chopper.
