29 CFR 1926.451(a)(6):Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design: a) Crosswinds Drive, Bellefonte, PA 16823 - Job Site - Employees were observed working from an elevated platform supported on the forks of a JLG/Gradall 534D6-42 Material Handler 16 feet above grade. The platform measured approximately 18 feet long x 47 inches wide, extending past the wheelbase and had not been designed by a qualified person, on or about March 27, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
