An employee working in an 8 ft excavation was exposed to serious struck by and or engulfment hazard, in that the spoils piles had not been placed at least two feet from the edge of the excavation.

An employee was exposed to serious struck by and or engulfment injuries, in that there was no protective systems in use for work being done in an 8-foot excavation. measuring 44 inches wide X 254 inches in length.
