29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. At the jobsite, employees were not wearing hardhats while working below other employees that were roofing a residential house, exposed employees to struck-by hazards from falling objects.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system. At the jobsite, employees removing old shingles and installing new underlayment to the roof of the house, were not wearing fall protection, exposing the employees to an unprotected fall hazard at a height of over 6 feet to the ground below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. At the jobsite, where employees performing roofing activities were exposed to fall hazards up to approximately 16 feet and did not demonstrate hazard recognition and avoidance to fall hazards at heights over 6 feet to the ground below.
