The employer did not ensure eye protection was utilized by employees operating pneumatic nail guns. Employees were exposed to struck-by hazards from ricochets, broken wood, and other debris.

The employer did not ensure employees on the roof utilized a fall protection system. The roof was measured and ranged in height of approximately 8-17 feet from ground to gutter board. The employees were exposed to fall hazards to the ground below.

A foldable multi position ladder was placed on the North side of the building for employees to access and egress the roof. The ladder side rails did not extend 3 feet above the upper surface exposing employees to fall hazards.
