On or about April 1, 2024, the employer did not ensure that employees were not using defective or damaged personal protective equipment in the following instances: a) In the kitchen / grill and fry areas, the thermal gloves had visible holes exposing employees to thermal burns. b) In the kitchen / grill and fry areas, the full face (burn guard/shield) shield was broken exposing employees to facial burns.
