29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) On April 2, 2024, and at times prior, Rstart LLC dba Plant Roofing and Solar, the controlling employer, did not ensure that subcontracted employees were protected from fall hazards while engaged in roofing operations at heights greater than 20 feet, without the benefit of fall protection systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. a) On April 2, 2024, Rstart LLC dba Plant Roofing and Solar, the controlling employer, did not ensure that that the portable ladders used by subcontracted employees to gain access did extend at least three feet above the landing surface, while engaged in roofing operations at heights greater than 20 feet. This condition exposed employees to fall hazards.
