1910.1200(h)(1) Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. a. On or about March 28, 2024, in the attic of the worksite: employees were exposed to fire and burn hazards when they applied a two-part spray foam insulation ( Utah Foam Products, Inc. Part A: 603-X-5425 Isocyanate, Part B: 603-X-5425 Polyol) in the attic of a residential home.

1926.503(c) "Retraining." When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee. Circumstances where retraining is required include, but are not limited to, situations where: a. On or about March 28, 2024, in the attic of the worksite: employees were exposed to an approximate 16 foot fall hazard while applying spray foam, in that, the gaps and holes in the exposed framework were not properly covered and fall protection was not used.
