29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) Workroom Floor, where the exterior loading dock vestibule doors were chained and locked. The loading dock vestibule doors were designated on the posted evacuation plan as an emergency exit and were equipped with an "Exit" sign.

1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source, distinctive in color or self-luminous or electroluminescent with a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2): a) Workroom Floor, where the vestibule door that was labeled "Employee Entrance" was equipped above the door with a metal "Exit" sign with black lettering on a white background that was not distinctive in color from a metal "No Exit" sign in the area.
