The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) Jobsite: Clay's Auto 309 Exchange Ave. Holmes, PA 19403 - On or about March 26, 2024, the employer did not ensure that an energy control procedure was in place, exposing employees to struck by and caught between hazards when maintaining equipment such as, but not limited to EEWH311A Air/Electric Tire Changer.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) Location: 309 Exchange Ave. Holmes, PA 19043 - The employer did not report an employee amputation within the required 24 hour timeframe upon being informed. An employee conducting maintenance on EEWH311A Air/Electric Tire Changer sustained an amputation on March 26, 2024. The Employer did not report to OSHA the amputation until March 29, 2024.
