Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a. On or about April 2, 2024, employees were exposed to molten materials and sparks while working at the melt extractor. The employer did not select and ensure employees utilized adequate protective clothing to protect from burns, such as, but not limited to; aluminized clothing designed to prevent molten material and/or sparks from burning through and/or entering clothing.

The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): a. On or about March 25, 2024, an employee was exposed to caught in hazards while performing cleaning and inspecting activities on the screener machine. The employer did not ensure that energy control application steps were implemented as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], control of gravitational energy by securing the drum [(d)(5)(i)] and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

On or about April 2, 2024, employees were exposed to flying metal and/or molten metal materials while working at the melt extractor. The employer did not provide guarding, such as barrier guards/shields, while employees worked at the equipment in operation. b. On or about April 2, 2024, employees were exposed to the rotating ME and wiper wheels on the melt extractor while operating the equipment. The employer did not ensure employees were guarded from contacting the pinch points created by the moving parts. c. On or about April 2, 2024, employees were exposed to the rotating ME and wiper wheels on the melt extractor while sanding the ME wheel by hand. The employer did not ensure employees were guarded from contacting the pinch points created by the moving parts.
