29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval: (a) Outside of building 11A; On or about April 3, 2024, and dates prior, the employer did not obtain written manufacturer approval from the forklift manufacturer before attaching trays (dog food trays) to forklifts, such as but not limited to, Toyota Model 50-8FGU25 propane forklifts, to empty collected waste extrusion material (dog food) into a dumpster.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a) Outside of building 11A; On or about April 3, 2024, and dates prior, forklifts, such as but not limited to, Toyota Model 50-8FGU25 propane forklifts, were not marked to identify the combined weight of the truck and the fork mounted tray (dog food tray) at maximum elevation, with its load centered.
