29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about April 1, 2024, the employer failed to ensure that effective guarding was in place at the point of operation of the dies on the Bending Machine (PKM0227). An employee was struck by the hydraulically operated dies when attempting to bend a small 1.5 inch round tube. PAKO, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1910.212(a)(1)] which was contained in OSHA Inspection #1481315, citation 1, item 1, which was affirmed as final order on October 15, 2020, with respect to the workplace located at 7615 Jenther Drive, Mentor OH 44060.
