In the assembly area, employees engaged in operating the Bridgeport Mill were exposed to struck-by hazards due to no chuck guard being provided. 2. In the drop hammer area, employees engaged in operating the 60X66 Drop Hammer were exposed to struck by and crushing hazards due to no point of operation guarding was provided for the second operator/helper.
