The Primeline Bending Machine (Large), Model# APH37175, Machine/Serial#705295619 pedal was not protected from fall objects which would cause the machine to activity without the aid of the operator.   Condition observed on or about April 10, 2024.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
