29 CFR 1910.217(b)(4)(i): The pedal mechanism shall be protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedal. a) Franklin Sheet Metal Co-Production floor: ) Franklin Sheet Metal Co-Production floor: Employees were exposed to amputation hazard(s) due to unintended operation of the foot pedal device being activated if an object falls on it. The Primeline Bending Machine (Large), Model# APH37175, Machine/Serial#705295619 pedal was not protected from fall objects which would cause the machine to activity without the aid of the operator. Condition observed on or about April 10, 2024. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
