29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) Jobsite located at or near 1131 Dorrance Street, Philadelphia PA- Front Scaffold- On or about April 5, 2024, the employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. Employees conducting siding activities were utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards or approximately 13 to 26 feet.  b) Jobsite located at or near 1131 Dorrance Street, Philadelphia PA- Rear Scaffold- On or about April 5, 2024, the employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. Employees conducting siding activities were utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards or approximately 13 feet.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access:  a) Jobsite located at or near 1131 Dorrance Street, Philadelphia PA- Front Scaffold- On or about April 5, 2024, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 4th and second level of the scaffold platform, thereby exposing employees to fall hazards of approximately 13 to 26 feet.  b) Jobsite located at or near 1131 Dorrance Street, Philadelphia PA- Rear Scaffold- On or about April 5, 2024, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 2nd level of the scaffold platform, thereby exposing employees to fall hazards of approximately 13 feet.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Jobsite located at or near 1131 Dorrance Street, Philadelphia PA- Front Scaffold- On or about April 5, 2024, the employer did not ensure that employees conducting siding activities were provided fall protection systems or guard rail systems, thereby exposing employees to fall hazards of approximately 13 to 26 feet.  b) Jobsite located at or near 1131 Dorrance Street, Philadelphia PA- Rear Scaffold- On or about April 5, 2024, the employer did not ensure that employees conducting siding activities were provided fall protection systems or guard rail systems, thereby exposing employees to fall hazards of approximately 13 feet.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) Jobsite located at or near 1131 Dorrance Street, Philadelphia PA- Front Scaffold- On or about April 5, 2024, the employer did not train each employee conducting siding activities on scaffolds, who were exposed to fall hazards between approximately 13 feet to 26 feet, to recognize the hazards associated with working from scaffolds to understand the procedures to control or minimize those hazards.    b) Jobsite located at or near 1131 Dorrance Street, Philadelphia PA- Rear Scaffold- On or about April 5, 2024, the employer did not train each employee conducting siding activities on scaffolds, who were exposed to fall hazards between approximately 13 feet, to recognize the hazards associated with working from scaffolds to understand the procedures to control or minimize those hazards.
