(a) On or about April 4, 2024, at the establishment located at 5005 S. 6th Street., Milwaukee, WI 53221, refresher training was not provided to employee when new forklifts were placed in service that resulted in changes that affected the safe operation/travel of the forklift, In-that, among other potential changes, the seat belts were not long enough to accommodate all forklift operators.
