29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) Near the intersection of Beaser Ave. and 3rd St. W; On or about April 2, 2024, the employer did not ensure that employees were protected from cave-ins using proper sloping methods, or alternative protective system(s), while working in an excavation with an approximate 9-foot depth, to install sanitary sewer drain piping in areas where the soil was previously disturbed.
